Procedure

1. Requesting an Accommodation

(Students should contact the Director of Special Services to request an accommodation.)

Employee

1.1 Discuss the need for an accommodation with the supervisor.

1.2 Identify and discuss with Human Resources an alternative posted position at CNM if the essential functions of the current position can no longer be performed.

Supervisor

1.3 Contact the appropriate Human Resources Representative.

1.4 Request, if appropriate, the nature of the medical condition, the accommodation requested, and the duration of the accommodation from the physician through the employee.

1.5 Determine, in conjunction with the Human Resources Representative, the nature and feasibility of the accommodation. 

Human Resources Representative / EEO Officer

1.6 Review and discuss the structural modification request if applicable.

EEO Officer or Designee

1.7 Review work site accommodations with the appropriate personnel, involving the Workers' Compensation Third Party Administrator if appropriate.

Supervisor

1.8 Order the necessary equipment or furniture for the accommodation with department head approval and based on the Physical Plant personnel's recommendation.

Human Resources Representative / Supervisor

1.9 Generate a letter to the employee stating that CNM has or hasn't approved the request for accommodation.

Human Resources Representative

1.10 File a copy of the letter in the employee's confidential official personnel file.
