Procedure

1. Contact and Referral

Employee – Self Referral

1.1  Contact the Human Resources Department at 224-4600 to obtain the Employee Assistance Provider’s (EAP) phone number.  Contact the EAP directly to schedule an appointment.

Supervisor – Formal Referral

1.2 Contact the Human Resources Department Representative and describe the problem.

1.3   Complete a Supervisor Referral Form and forward it (along with supporting documentation) to the Human Resources Department.  An appointment will be scheduled, and the employee will be required to attend.

Supervisor – Informal Referral

1.4   Discuss concerns with the employee and suggest or recommend that s/he contact the EAP.

Supervisor – Emergency Referral

1.5 Contact the Human Resources Representative or EAP representative and describe the problem.

1.6 Complete a Supervisor Referral Form and forward it to the Human Resources Department.

Supervisor – Mediation Referral

1.7 Contact the Human Resources Representative or EAP representative and describe the problem.

1.8 Complete a Supervisor Referral Form and forward it to Human Resources.

2. Documentation

Supervisor

2.1 Document in writing the specific date, time, place, event and action taken.

2.2 Coordinate with Human Resources Representative for follow-up.
