Procedure

1. New or Rehired Employees

Supervisor

1.1 Notify the Human Resource Technician prior to the new or rehired employee's hire date to schedule the employee for the next available Phase I Orientation session.

1.2 Review Phase II Orientation Checklist to guide discussion with the employee on their first day on the job and provide the Phase II Orientation to the employee.

Employee

1.3 Attend Phase I orientation at the scheduled time.

1.4 Complete the employment related forms and enroll in benefits if desired.

1.5 Contact Staff Training and Development by phone 224-4706 or email ajacobs@tvi.edu to schedule other compulsory training, including Sexual Harassment Awareness and Hazard Communications training as soon after the start of their employment as possible.

Human Resources

1.6 Conduct the Phase I Orientation, distribute the Employee Orientation packet and review payroll, benefits and general Institute policy information.

1.7 Ensure that the employment related forms are complete and disseminate to the departments in accordance with Table 1.

1.8 Prepare Personnel Data Form (PDF) and submit to Payroll Department.

1.9 Coordinate scheduling of other compulsory training.

Payroll

1.10 Process Personnel Data Form, I-9 (effective 1/1/2001, I-9 forms will be kept in Human Resources), W-4 and direct deposit, ERA application in accordance with Payroll departmental procedures.

2. Status Changes

Supervisor

2.1 Notify the Human Resource Technician for their department of the employee's change of status.

2.2 Provide Phase II Orientation to cover information related to job duties and expectations.

Human Resources

2.3 Notify employee of eligibility to enroll in benefits due to change of status.

Employee

2.4 Request supervisory approval to attend Phase I orientation to review benefits information, if desired, or obtain information from Human Resources Technician by phone or in person.

Human Resources

2.5 Provide information regarding benefits eligibility.

Employee

2.6 Enroll in benefits if desired.

Human Resources

2.7 Prepare personnel data form (PDF) and submit to Payroll Department.

2.8 Process benefit enrollment applications in accordance with Human Resources Department procedures.
